The requirement will include a base period plus three 3-month option periods. The period of performance will be as follows: Base Period: 01/01/2022 - 03/31/2022 1st Option Period: 04/01/2022 - 06/30/2022 2nd Option Period: 07/01/2022 - 09/30/2022 3rd Option Period 10/01/2022 - 12/31/2022 The North American Industry Classification System or NAICS code is 62111 with a small business size standard of $12.0 M. The Request for Quotations No. 75H71022Q00005 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 11/15/2021. Evaluation factors may include: 1. Ability to manage complex patients 2. Contractor’s experience of three (3) years beyond residency training. 3.
