The Request for Quotations No. 75H71021Q00049 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 08/30/2021. Evaluation factors may include: 1. Ability to manage complex patients 2. Contractor’s experience of three (3) years beyond residency training. 3. Contractor’s Licenses/Certifications (BLS, medical license, Diplomas etc.), Board Certified/Board Eligible & Valid Driver's License 4. Competency in Family Medicine and Palliative Care 5.
