The Indian Health Service, Gallup Service Unit, located in Gallup, New Mexico, 87301 has an upcoming requirement for five (5) Full-Time Non-Personal Healthcare Services for Physicians in Anesthesia Department. The North American Industry Classification System or NAICS code is 62111 with a small business size standard of $12.0 M. The Request for Quotations No. 75H71021Q00048 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 08/30/2021. Evaluation factors may include: 1. Provider shall provide professional medical services in the inpatient and outpatient setting. 2. Contractor’s experience of twelve (12) months (may include training program) 3. Contract Requirements and must show the following: -Board Certified/Board Eligible in Anesthesiologist -Competency in Anesthesiologist/CRNA -Confidentiality of patient care and records -Credentialing by Medical Staff -GIMC Contractor’s Licenses/Certifications, PALS (required), BLS (required), ACLS (required), Valid Driver's License 4. Residency in Anesthesiologist 5. Two (2) References of the Contractor 6. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile. 7. Profiles being submitted for start date of September 2021, must be available to start as soon as possible. The remains positions must be available to start 01/01/2022. 8. Price Quotes will be due on or by 09/03/2021. This acquisition is unrestricted.
