The Indian Health Service, Gallup Service Unit, located in Gallup, New Mexico, 87301 has an upcoming requirement for one (1) Full-Time Non-Personal Healthcare Services for Physicians in Internal Medicine Department. The requirement will include a base period plus 3-month option periods, The period of performance will be as follows: Base Period: 01/01/2022 – 03/31/2022 1st Option Period: 04/01/2022 – 06/30/2022 2nd Option Period: 07/01/2022 – 09/30/2022 3rd Option Period: 10/01/2022 – 12/31/2022 The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $12.0 M. The Request for Quotations No. 75H71022Q00012 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 12/07/2021. Evaluation factors may include: 1. Contractor shall demonstrate: a) Competence in Internal Medicine or Family Medicine. b) Competence in adult medicine procedures. c) Confidentiality of patient care and records. d) Successful application for credentialing by the medical staff of GIMC. e) Maintenance of professional certification through participation in appropriate programs for continuing medical and pharmaceutical education. 2. Contractor’s experience of twenty-four (24) months. 3. Contractor’s Licenses/Certifications (ACLS, medical license, Diplomas etc.) 4. Current Board Certified eligibility or certification by the American Board of Internal Medicine or Family Medicine. 5. Completion of an Internal Medicine or Family Medicine residency program. 6. Valid Driver's License 7. Two (2) References of the Contractor 8. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile 9. Current Copy of Certificate of Insurance 10. Submittal of Option CLIN 11. Price Quotes will be due on or by 12/14/2021. This acquisition is unrestricted.
