The Billings Area Indian Health Service (BAIHS) is requesting quotations in response to Request for Quotation (RFQ)-75H709-BAIHS25Q-012 to acquire Prescription Eyeglasses for the BAIHS Service Units in the Billings Area in the state of Montana. The procurement is conducted under Simplified Acquisition procedures pursuant to the authority of Federal Acquisition Regulation (FAR) Subpart 13.5, Simplified Procedures for Certain Commercial Items. The Billings Area Indian Health Service (BAIHS) intends to award a Blanket Purchase Agreement, contract for 12 months with four 12-month options. The Purchase Referred Care Offices at the BAIHS Service Units will authorize eyewear purchases by obligating the funds on the IHS-T-843-1A Purchase-Delivery Order for Health Services. Prices are required for all items in Attachment A and should not be more than the Contractor's usual customary fee. All other items should be at standard market value
