The Indian Health Service (IHS), Albuquerque Area office (AAO), Santa Fe Indian Health Center (SFIHC) has a requirement for an Automatic Pharmaceutical-Dispensing Equipment Lease and Support for the Santa Fe Indian Health Center, Santa Clara Health Center and Cochiti Health Clinic. This is a combined synopsis for commercial items prepared in accordance with format in FAR 12.6, as supplemented with additional information included in this notice. The solicitation number is 75H70723Q00146 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2023-04. The associated North American Industrial Classification System (NAICS) code for this procurement is 532490 with a business size standard $40.0. This requirement is Set-Aside for Small Business and only quotes from qualified offerors will be considered. If IHS does does not receive enough competition and/or reasonably priced offers from Small Business Set-Aside, IHS will then evaluate quotes and consider for award to other Small Business concerns. Any acquisition resulting from this cascading set-aside will be from as follows: First Tier: Small Business; Second Tier: Service Disabled Veteran-Owned Business; Third Tier: Women-Owned Small Business; Fourth Tier: HUBZONE; Fifth Tier: All Other Large Business Concerns. ONLY ONE AWARD WILL RESULT FROM THIS SOLICITATION. FOB Destination shall be for the following Albuquerque Area Indian Health Service facility: SANTA FE INDIAN HEALTH SERVICE (SFIHC), Attn: Pharmacy Department, 1700 Cerrillos Road, Santa Fe, New Mexico 87505. This solicitation will result in a Single Award with a Base + 4 Option Periods from the date of award baed on Satisfactory Performance ratings. State Taxes for labor/services costs (The Indian Health Service is exempt from New Mexico State Taxes for materials and supplies). This solicitation requires registration with the sam.gov prior to award, pursuant to applicable regulations and guidelines.
