The Albuquerque Area Indian Health Service, Santa Fe Indian Health Center, has a requirement for Fabrication of Eye Glasses. This is a combined synopsis for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 75H70722R00010 and is issued as a Request for Proposals (RFP), unlessotherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06. The associated North American Industrial Classification System (NAICS) code for this procurement is 339115 with a small business size standard of 1,000. This requirement is a Set-Aside for Small Business Indian Firms and only proposals from qualified offerors will be considered. If we do not receive enough competition and/or reasonably priced offers from Small Business Indian Firms set-aside, we will evaluate proposals and consider for award to other Small Business concerns. Any award resulting from this cascading set-aside will be from such a concern as follows: First Tier: Small Business Indian Firms; Second Tier: Small Business; Third Tier: Service Disabled Veteran-Owned Business; Fourth Tier: HubZone; Fifth Tier: All Other Large Business Concerns. Only one award will result from this solicitation.
