This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6-Streamlined Procedures for Evaluation and Solicitation for Commercial Items-as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. This is a 100% Small Business Set-Aside. This solicitation is being issued using Simplified Acquisition Procedures under the authority of FAR Subpart 13.5 Simplified Procedures for Certain Commercial Items. This solicitation is a Request for Quote(RFQ) # 75H70525Q00007, this number must be included in all correspondence and proposal submission. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2024-07 Effective 09/30/2024. The Department of Health and Human Services/Indian Health Service has a requirement for preventive maintenance and repair services for X-ray equipment for the Bemidji Area Indian Health Service.
