The Bemidji Area Indian Health Service is requesting proposals for Comprehensive Healthcare Services for urban Native American Beneficiaries, in accordance with P.L. 94-437 Title V of the Indian Health Care Improvement Act, as amended, for the continuation of Urban Indian Health Programs. The services are for a health care system with limited direct services (outpatient clinic, mental health, and pharmacy services) that supports an extensive outreach and referral to the eligible American Indian/Alaskan Native (AI/AN) population residing within the Chicago Illinois metropolitan area. The period of performance will be October 01, 2021 through September 30, 2022 with an option to extend the contract for four (4) one-year periods. Services to be provided include Community Health Representative services, limited clinical services medical, nursing, pharmacy, laboratory, medical records, dental, and optometric), outreach and referral services, and limited transportation services. This solicitation is authorized and governed by the Indian Health Care Improvement Act, no outside parties shall be provided the solicitation.
