This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. b. Bemidji Area, Indian Health Services (IHS) intends to award a Firm Fixed-Price, Commercial Item, Purchase Order, in response to Request for Quote (RFQ) 75H70520Q00025 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06, dated 05/06/2020 (c) This RFQ is issued as a 100% Small Business Set Aside and the associated NAICS code is 238220 with a small business size standard of $16.5 million. (d) Description of Service: Provide all personnel, transportation, equipment, tools, materials, supervision, and other items and services necessary to complete the required service IAW Scope of Work. The service will provide heating, ventilation, and air conditioning (HVAC) equipment maintenance at the (3) sites listed below. Place of Performance: White Earth Health Center, 40520 County Highway 34, Ogema, MN 56569 Naytahwaush Health Center, 238 Church Street, Naytahwaush, MN 56566 Pine Point Health Center, 47520 Mashkiki Street, Ponsford, MN 56575 (e) The period of performance is Base plus (4) Option Years IAW Schedule B (f) The provision at 52.212-1, Instructions to Offerors – Commercial Items (Mar 2020), applies to this acquisition.
