This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. Solicitation number 75H70421R00004 applies, and is issued as a Request for Proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-02. This requirement is a total Small Business set-aside and the associated NAICS is 541690 – Other Scientific and Technical Consulting Services with small business size standard of $15M. See the attached Additional Terms and Conditions – Appendix F-Offeror’s Pricing Sheet-Schedule of Charges. This Time-and Material (T&M) contract is to support the Indian Health Service (IHS), Joslin Vision Network, Tele-Retina (IHS/JVN-TR) program. The IHS needs to maintain ongoing operations, development, implementation, and support of the Indian Health Service Tele-Retina Program (IHS-TR) activities in Indian country with services and equipment in accordance with the equipment specification and tasks and in accordance with Performance Work Statement (PWS) and solicitation. See the attached Additional Terms and Conditions – Section C, for further details. See attached – Appendix A – Schedule of Deliverables and IHS Service Locations. The provision at 52.212-1, Instructions to Offerors – Commercial Items (Jun 2020) applies to this acquisition. 52.212-2, Evaluation -- Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Technical Capability and Approach; 2) Key Personnel; 3) Recent and Relevant Past Performance; and 4) Price. Technical Factors, when combined, are more important than price. All non-price factors are of equal importance. See the attached Additional Terms and Conditions – Sections L & M, for details. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Nov 2020), with its offer. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov.
