The purpose of the project is to provide diabetes surveillance, technical assistance, education and training, and Area-wide outreach activities to support the Portland Area. This will be accomplished by providing professional expertise and management of the tasks detailed in the scope of work. It will require as many as four specialists that can provide services for varying amounts of time throughout the year. The quality, quantity, and timeliness of performed services will make a significant impact on the DDTP mission and overall program performance and will ultimately affect key stakeholders that include IHS leadership, as well as the Portland Area healthcare programs.
