This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 75H704121Q00035 applies, and is issued as a Request for Quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-07. (iv) This requirement is a total Woman Owned Small Business set-aside and the associated NAICS is 541690 - Other Scientific and Technical Consulting Services, with a small business size standard of $16.5M. (v) A Firm-Fixed Priced Purchase Order is be awarded as a result of this RFQ. The Purchase Order will include a one-year Base Period and four (4) one-year Option Periods. Quoters are to provide a firm-fixed price quotation for each contract period. Unit price for the CLINs shall include any related travel and materials costs. See the attached Solicitation Pricing Table for the pricing structure. Please complete and submit the Solicitation Pricing Table as part of any response to the RFQ. (vi) The purpose of this fixed price purchase order is to obtain the services of one (1) contractor to provide one (1) employee for non-personal professional oral epidemiology surveillance services. These services shall be provided in support of the Indian Health Service’s Office of Clinical and Preventive Services – Division of Oral Health. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced.
