End of Amendment A0001 ######################################################################################################## This is a Solicitation Notice for solicitation number 75H70125Q00019, Fort Thompson Health Center X-Ray Replacement Project, a construction project for the Indian Health Service (IHS), Division of Engineering Services (DES) prepared in accordance with FAR Part 36. The purpose of this project is for the installation of supporting utilities for the new Philips X-Ray unit at the Fort Thompson IHS Clinic. The X-Ray unit was already purchased by the service unit and it is not included in this project. The radiology unit at the Fort Thompson IHS Clinic currently has an old GE X-Ray unit. These units will be replaced by a new Philips X-Ray machine. The new machines require different support structure, electrical, and a change in flooring layout. This project will be procured as a 100% Small Business set-aside solicitation under North American Industry Classification System (NAICS) 236220, Commercial and Institutional Building Construction. The associated size standard for this procurement is $45 million. Please review all attachments to this notice for details. SCOPE OF WORK: See Section C of the attached solicitation for details. LOCATION OF PROJECT: Fort Thompson, SD CONSTRUCTION DURATION: Period of Performance is 180 calendar days after issuance of a Notice to Proceed. CONSTRUCTION MAGNITUDE: In accordance with FAR 36.204, the magnitude of this construction project is anticipated to be between $100,000.00 and $250,000.00. SITE VISIT: See Section L.1. FAR 52.236-27 SITE VISIT (CONSTRUCTION)(FEB 1995) ALTERNATE I (FEB 1995) CONTRACT TYPE: The IHS intends to award a Firm Fixed Price (FFP) construction contract in support of this requirement. This solicitation will be procured as a Request for Proposal (RFP) in accordance with FAR Part 13 and 36 procedures. QUOTE REQUIREMENTS: See Sections L&M of the solicitation. QUOTE DUE DATE: The quoter’s proposal shall be submitted electronically no later than the due date and time indicated in block 13 of the SF 1442, unless changed by amendment. EVALUATION: In accordance with FAR 13.106-2, the evaluation for this award will be based on Price, Technical Experience Narratives Similar in Scope within the past 5 years and the quoter’s compliance with the terms and conditions for this request for quotes; see Section L. Prices quoted must reflect the quoter’s understanding of the requirement, ability to perform, and be considered fair and reasonable to the Government. Price is the discriminating factor for award among eligible quoters. The total firm fixed price listed in block 17 of the SF1442 and on the attached price schedule in Section B shall be utilized for the overall price evaluation and to determine the lowest priced quote. If the entity responding to this sources sought is submitting experience information from any entity that does not match the name, CAGE code, and/or UEI exactly as stated on the submitting entity’s SAM record (e.g. affiliates, subsidiaries, parent companies, sister companies, etc.) the submission shall clearly demonstrate how that entity will have meaningful involvement in the performance of the contract and specifically address which resources would be committed to the contract. Indian Small Business Economic Enterprises (ISBEEs) as well as other small business are highly encouraged to respond to this notice. However, firms should understand that generic capability statements are not sufficient for effective evaluation of their capacity and capability to perform the work required.
