This Request for Quotation (RFQ) (No. 75H70120Q00054) is issued to request price quotes for the reconfiguration of airflow in the Fort Hall Health Clinic’s open operatories as specified in Section C of the attached soliciation. This solicitation is 100% set aside for small business under NAICS 238220, Plumbing, Heating, and Air?Conditioning Contractors, with a size standard of $16.5M. In accordance with FAR Part 13, the Indian Health Service (IHS) intends to award a firm fixed price (FFP) purchase order to the responsible offeror, quoting the lowest price. PROJECT TITLE: Fort Hall Dental Airflow Reconfiguration Services PROJECT LOCATION: Not-Tsoo Gah-Nee Indian Health Center 33 North Mission Road Fort Hall, ID 83203 (Bingham County) PERIOD OF PERFORMANCE: The performance period for this contract is 20 calendar days from issuance of the Notice to Proceed (NTP). REQUIRED DOCUMENTATION: The quote shall include the following: The solicitation number and acknowledgement of any solicitation amendments issued; The offeror’s name, address, DUNS number, CAGE code; The name, title, email, phone number, of the individual authorized to sign any resulting contract; Confirmation of HVAC license and a minimum 5-years of experience with commercial HVAC and ductwork installation; Confirmation of certification with the Associated Air Balance Council (AABC) or the National Environmental Balancing Bureau (NEBB); and A price to perform the entire work as specified in the SOW within Section B Price Schedule (Section J, Attachment J01) On a separate page, include a breakdown of the pricing in sufficient detail for evaluation (see Section B, paragraph B.4). NOTE: In order the be eligible for award, the contractor (or subcontractor performing the work) must (1) hold an HVAC license, (2) have a minimum of 5-years of experience with commercial HVAC and ductwork installation, and (3) be certified with the Associated Air Balance Council (AABC) or the National Environmental Balancing Bureau (NEBB).
