Notice of Intent to Sole Source to The Joint Commission The Effects of Organizational Culture and related factors on Healthcare Worker (HCW) Well-being and Patient Safety: A Scoping Review of Available Evidence The Centers for Disease Control and Prevention (CDC), Office of Acquisition Services (OAS) on behalf of NIOSH, intends to award a sole source firm fixed priced simplified purchase order to obtain services to conduct a scoping review that will focus on examining the effects of organizational culture and related factors on healthcare worker well-being and patient safety. The initial scoping review will examine aspects of organizational culture such as safety culture, safety climate, leadership, teamwork, communication, and learning environment. Interventions in these areas will be identified where available and examined for their impact as well as any additional research or evaluation that may enhance the effectiveness or applicability of the intervention. This work is a continuation of collaborative efforts between The Joint Commission and the NIOSH NORA Healthcare and Social Assistance Sector Program. In 2012, The Joint Commission published a monograph on improving patient and worker safety. The purpose of the scoping review is to build on this foundational work by examining specific organizational culture interventions that can improve both patient and worker safety. The main objective is to to identify and review intervention studies, but such studies are likely to be relatively few in number. If only very few intervention studies are identified, the focus of the scoping review may widen to include ecological studies documenting relationships between organizational culture and related factors and healthcare worker well-being and patient safety that might inform development of an evidence-based conceptual framework for development of interventional trials. This initial scoping review will focus on findings that can be used to strengthen healthcare leadership’s commitment to worker well-being, identify research or evaluation gaps that might inform organizational culture and related interventions, and facilitate improvements in safety for both workers and patients. This work would be an important contribution to current national efforts such as the clinician burnout activities being conducted by the National Academy of Medicine and the patient safety/worker safety work being done by OSHA and the Institute for Healthcare Improvement. Scoping reviews are particularly appropriate when an overall body of literature has not yet been comprehensively reviewed or when it displays a complex or heterogeneous nature (Peters et al., 2015). Scoping reviews can be useful in determining the merit and requirements of completing a full systematic review; they also can serve to summarize research findings, identify research gaps, and provide direction for future research. Scoping reviews, while different from systematic reviews, still require clear and transparent methods. Guidance for conducting scoping reviews has expanded considerably during the last decade (e.g., Arksey & O’Malley, 2005; Colquhoun et al., 2014; Munn et. al., 2018; Hempel, Xenakis, & Danz, 2016; Tricco et. al., 2018). The Contractor shall furnish all labor, equipment, materials, and supplies necessary to perform the tasks that follow. Some tasks can be performed concurrently, where appropriate. SECTION 4 – TASKS TO BE PERFORMED Task 1: Identify Study Personnel and Establish Subcontracts or Agreements • Subtask A: Within 2 months of contract award the Contractor will identify key study personnel to participate in this effort. • Subtask B: The Contractor will enter into any necessary agreements with the identified study personnel. • Subtask C: Within 2 months of contract award, the Contractor will provide a list of all study personnel, their contact information and affiliations. • Subtask D: Within 3 months of contract award the Contractor will hold a web-based meeting with NIOSH to introduce study team Task 2: Conduct Search of Literature • Subtask A: Within 3 months of contract award the Contractor will hold a web-based meeting with NIOSH to finalize the research question that the scoping review will address to the mutual agreement of all parties. • Subtask B: Within 4 months of contract award, the Contractor will hold a web-based meeting with NIOSH to finalize the search parameters for the scoping review to the mutual agreement of all parties. This will include inclusion and exclusion criteria as well as specifications for the population, intervention, comparison, outcome (PICO) as well as any application of PRISMA criteria. • Subtask C: Within 5 months of contact award, the Contactor will initiate a search of relevant literature. This search may involve both searches of electronic databases, but also hand searches of reference lists, key journals, and grey literature. The search strategy may be revised at this time and may require a web-based meeting with NIOSH to change the research question or search criteria. • Subtask D: The Contractor will develop a database of literature, using software also available to NIOSH (ie: Excel, Access, EndNote), to include all papers and articles that fit the search criteria. • Subtask E: Within 8 months of contract award, the Contractor will produce an initial (interim) database of relevant literature. Task 3: Apply Inclusion and Exclusion Criteria • Subtask A: Within 8 months of contract award, the Contractor will identify at least 2 individuals to be involved in selecting articles or papers at each stage of the review. NIOSH staff may be available to assist in this review. Disagreements regarding inclusion or exclusion must be resolved by the two reviewers through consensus. • Subtask B: The Contractor will apply relevant inclusion and exclusion criteria to review identified articles and papers in an incremental fashion: 1. Review of titles 2. Review of abstracts 3. Review of full manuscripts • Subtask C: Within 12 months of contract award, the Contractor will document the process of including or excluding literature in a PRISMA flowchart to be incorporated in the final scoping review manuscript. • Subtask D: Within 12 months of contract award, the Contractor will produce a final database of selected papers and articles to be included in the scoping review. Task 4: Create Abstraction Form • Subtask A: Within 6 months of contract award, the Contractor will produce a draft abstraction form to extract relevant data from papers and articles selected for inclusion in the scoping review. At this time, consideration of the final scoping review manuscript, including possible tables or figures, should impact the information collected in the abstraction form. • Subtask B: Within 8 months of contract award, the Contractor will hold a web-based meeting with NIOSH to finalize the abstraction form to the mutual agreement of all parties. • Subtask C: Within 9 months of contract award, the Contractor will produce a final abstraction for reflecting all comments and edits from NIOSH. Task 5: Abstraction of Relevant Data • Subtask A: Within 15 months of contract award, the Contractor will identify at least 2 individuals to be involved in completing one abstraction form for each article or paper selected for inclusion in the scoping review. Disagreements regarding abstracted data must be resolved by the two reviewers through consensus. NIOSH staff may be available to assist in this abstraction. • Subtask B: Within 15 months of contract award, the Contractor will produce a database of information extracted from the articles and papers selected for inclusion in the scoping review using software also available to NIOSH (ie: Excel, Access, EndNote). Task 6: Summarizing Results in a Submitted Manuscript and Presentation • Subtask A: Within 18 months of contract award, the Contractor will produce a draft manuscript summarizing the scoping review process and results. This draft will be submitted to NIOSH for review. • Subtask B: Within 20 months of contract award, the Contractor address all relevant comments and edits received from NIOSH and will submit the draft manuscript to an academic journal for publication. The journal will be selected in consultation with NIOSH and to the mutual agreement of all parties. • Subtask C: Within 24 months of contract award, the Contractor will present the principal findings of the scoping review to NIOSH and other identified stakeholders via a web-based meeting. This contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b) (SAP) and 41 U.S.C. 3304(c)(1). Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive quotations; however, all quotations/responses received within seven (7) days of the issuance of this notice will be considered by the government. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
