NOTICE OF INTENT TO SOLE SOURCE: This requirement is for (supplies/services) in which the Government intends to solicit and negotiate with only one-source under the authority of FAR 13.106-1(b) (SAP) and 41 U.S.C. 3304(a)(1). Interested persons may identify their interest and capability to respond to the requirement. This notice of intent is not a request for competitive quotations; however, all responses received within seven (7) days of the issuance of this notice will be reviewed. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Any response should be emailed to the contract specialist at ycf1@cdc.gov by 1:00 p.m. Eastern Standard Time (EST) on June 29, 2020. SCOPE OF WORK The Contractor shall conduct 3 internet-based surveys, each in up to 10 states to be identified by CDC. Each survey will include approximately 25-30 individual questions related to chronic diseases and conditions (e.g., conditions such as pregnancy, diabetes and cancer; risk factors and behaviors such as tanning and toothbrushing; and medical or management factors such as care-seeking and participation in lifestyle change programs). Once it has been determined which questions and populations will be addressed, the Contractor will perform the data collections. Depending on the specific needs of each survey, the Contractor may use existing panels of construct new panels. The Contractor will provide CDC with results that are designed to be valid, stable, and representative of the adult population of each participating state as well as results representative of the subpopulations of interest within each state, with approximately 400-500 adult respondents per state. The Contractor will additionally provide CDC with a dataset and corresponding codebook of data collected for each target population group.
