This solicitation notice is a request for proposals “Non-Personal Service” contract for one (1) Ambulatory Care Registered Nurses to provide services at 4 Corners Regional Health Care Center, HCR 6100, U.S. Hwy 160 & Route 35, Red Mesa, AZ. Services shall be performed in accordance with the attached Performance Work Statement (PWS). The Period of Performance will be issued for 6 months each will have 1) Base Period 2) with an Option 1 Period. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the nurse will be released from contract assignment. The North American Industry Classification System or NAICS code is 561320 with a small business size standard of $30M. This solicitation is using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures. EVALUATION CRITERIA: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant factors when combined are significantly more important than cost or price.
