Monthly Diabetic foot clinic onsite, one day a month. 2. One pair DM shoes 3. Maximum, 3 DM inserts (vi)Description of requirements The Catawba Service Unit will contract with a state licensed Orthotist/Pedorthist to provide a monthly foot clinic to our Diabetic population at the Catawba Service Unit. The contractor will evaluate and design a treatment plan for each individual who attends the clinic. The plan may include; extra depth or custom shoes and /or molded inserts. One pair of shoes and up to three inserts will be provided annually, unless otherwise indicated. Measurements will be taken at the time of the evaluation and prescribed device(s) ordered. The device(s) will be available for pick up the following month. The contractor, along with the Service Unit will plan and provide group and /or individual education as it relates to Diabetic foot care. The deliverables are custom prescribed orthotic footwear. One pair of shoes and up to three pair of inserts will be provided to each patient annually. To qualify for footwear coverage, Medicare beneficiaries must have diabetes plus one of the following conditions: Neuropathy with evidence of callus, previous or current ulcer, previous or current pre-ulcerative callus, previous, amputation, foot deformities, or poor circulation. The necessary information will be submitted on the form completed by the service unit M.D. The proposed number of patients to be serviced is approximately 60. The contractor shall be licensed in the state of South Carolina. The contractor will be required to enter into an Business Associate Agreement with IHS, and pass an IHS background check and credentialing. (vii) Performance One Day a month onsite at the Catawba Service Unit. The period of performance shall be awarded for the base year plus four option years.
