This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-24-Q-0173. This procurement is 100% Indian Owned set-aside under NAICS code 221320 with a size standard in millions of dollars $35.0. The Contractor shall furnish leachfield chamber supplies for Lawton Indian Health Service as requested in the attached statement of work. The period of performance requested for this requirement is: FOB 30 DAYS from date of award. Estimated price range of this solicitation: Less than $25,000. There is no official site visit. Questions are due no later than July 17, 2024 5:00 p.m. CST. The closing date for receipt of quotes is July 24, 2024 COB CST. THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote with attached bid sheet B) Technical specifications of the statement of work, with acceptable materials list to be used. C) Submit two (2) Relevant past performance of this type of work, shall provide organization’s name, point of contact, summary of project and award amount. D) Submit Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form (attached) Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS.
