This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 246-22-Q-0120 this solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-6. This is 100% Set Aside under the Buy Indian Act, NAICS code 334510; the small business size standard is 1,250 employees. The Anadarko Indian Health Center requires a Digital Mobile XRay Equipment to include delivery, set up and installation in accordance with the attached specifications cited in the Scope of Work. The provision at 52.212-1, Instructions to Offerors-Commercial is applicable. The evaluation will be based on acceptable/non-acceptable basis. Indian Health Service shall not consider your quote for award if you fail to provide a quote containing: Firm Fixed Price Technical Specifications of your quote along with country of origin (2) verifiable recent past performance that includes similar experience in equipment similar to the Specifications in the Statement of Work. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted quotes. The Government is not responsible for locating or securing any information which in not identified in the quote. The Government reserves the right to make an award without discussions. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
