Additional Statement of work information and drawings (see attachments). The site visit has been moved to 2/10/2022 at 10:30. The bid due date has been moved out to 3/4/2022. This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-22-Q-0047. This procurement is 100% small business set-aside under NAICS code 238220 with a size standard in millions of dollars $16.5. Contractor shall remove and replace air handling unit 2A with new equipment providing the ability to properly control humidity as well as maintain proper temperature and pressure relationships in a sterile supplies storage area in accordance with the statement of work at the Claremore Indian Hospital. The period of performance for this requirement is 90 calendar days from Notice to Proceed. Site visit is scheduled for February 10, 2022. Questions are due no later than February 11th 2022 5pm CST. The closing date for receipt of quotes is March 4, 2022 COB CST. The offer shall be received by email to jeffrey.earnest@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address, no later than 5:00 p.m. CST, on March 4, 2022. THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote with attached bid sheet B) Technically specifications of the statement of work, acceptable materials list to be used. C) (2) Relevant past performance of this type of work, shall provide organization’s name, point of contact, summary of project and award amount D) Bid bond form (SF24) attached is required at $35,000. If over $35,000 and selected as the acceptable vendor proposal, performance (SF25) and payment (SF25A) bonds shall apply Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
