**Amendment A-02: Amended solicitation to incorporate site visit sign-in sheet, Q&A's and updated drawing. See updated attachment **Correction: Site Visit will take place on Tuesday Dec. 14th at 10:30** This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-22-Q-0018. This procurement is 100% small business set-aside under NAICS code 332321 with a standard business size of 750 employees. Contractor shall provide a firm, fixed-price quote to remove entry and interior doors and replace with new automatic touchless doors at the Wewoka Indian Health Center. The period of performance for this requirement is 90 days from the Notice to Proceed. Site visit is scheduled for Tuesday Dec. 15, 2021 at 10:30am CST. Questions are due no later than Dec. 17, 2021 at 5pm CST. *Due to the pending CR end date, solicitation dates are subject to being rescheduled. The closing date for receipt of quotes is Dec. 29, 2021 at 5pm CST. Include contracting organization, contract POC (phone number and email) and contract amount. Proposals shall be evaluated on an acceptable/non-acceptable basis. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for capability statement and past-performances beginning with the lowest priced quote 3. This process shall proceed until an acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE SCOPE OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR PROVIDING THE LOWEST REASONABLE PRICE, MEETING ALL TECHNICAL SPECIFICATIONS AND PAST-PERFORMANCES.
