This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 246-21-Q-0140 this solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-5. This is 100% Set-aside for Women Owned Small Business, NAICS code 334290; the small business size standard is 750 Employees Contractor shall provide a firm, fixed-priced all inclusive quote to provide Brand Name or Equal – Axis Network Cameras and Accessories for the Lawton Indian Hospital. The provision at 52.212-1, Instructions to Offerors-Commercial is applicable. Evaluation Criteria: The evaluation will be based on Lowest Priced Technically Acceptable. Must meet all the requirements as stated in the Specifications to be considered technically acceptable. The Indian Health Service will not consider your quote for award if you fail to provide Technically Acceptable- The offeror’s response shall provide conclusive evidence or support their ability to provide Network Cameras in accordance with the Specifications. Registration in the System for Award Management (SAM) database at https://www.sam.gov. Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database as a WOSB. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.
