This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 246-21-Q-0072 this solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-5. This is Full and Open Competition, NAICS code 621512; the small business size standard is $16.5M. The Lawton Indian Hospital would like to lease a Mobile Toshiba Aquilion 64 or equivalent whole-body Computed Tomography System. The Scope of Work is attached. System shall be onsite August 29, 2021 with anticipated removal on October 15, 2021. The provision at 52.212-1, Instructions to Offerors-Commercial is applicable. The evaluation will be based on technical and past performance. Must meet all the requirements as stated in the Statement of Work specifications to be considered technically acceptable. Submit three references with current phone numbers. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.
