This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 246-21-Q-0025 this solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-4. This is 100% Set aside for Small Business, NAICS code 812332; the small business size standard is $41.5M. Laundry Services and Linen Rental for the Lawton Indian Hospital. The Statement of Work is attached. The provision at 52.212-1, Instructions to Offerors-Commercial is applicable. The evaluation will be based on past performance. The offeror must submit past performance information covering the past three (3) years. The following areas will be reviewed and rated: Quality of service (meeting the performance standards established in the work statement statement); Timeliness of performance (ability to meet schedule); and Management (offeror’s general businesslike concern for the interest of the customer) One of the following two past performance ratings shall be assigned to that offeror. ACCEPTABLE: Based on the offerer’s past performance record, the government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown. UNACCEPTABLE: Based on the offeror’s performance record, the government has no reasonable expectation that the offeror will be able to successfully perform the required effort. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
