This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-21-Q-0010. This procurement is full and open competition under NAICS code 811213 with a standard business size of $11.0 million. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-04, effective January 25 2021. The Indian Health Services intends to make a single award for Secure Data Exchange and Clearinghouse Services for the Oklahoma Area Indian Health Services. Demonstration sessions to include overview of model/solutions shall be included during this solicitation. Times have been set aside during March 29th and March 30th. Due to Covid restrictions, all demonstrations shall be remote. Time length is set for no longer than 30 minutes. STATEMENT OF WORK ATTACHED CLAUSES INCORPORATED BY FULL TEXT: Please note that due to character limitation in FedBizOpps, full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulations (HHSAR) can be accessed on the Internet at http://www.farsite.hill.af.mil. In support to the information in FAR 52.212-1 & FAR 52.212-2 (A.10 and A.11 on Clauses), prospective contractors shall provide the following: (1) Dun and Bradstreet Number (note: Contractors must be registered in the System for Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. Payments will be made using Electronic Funds Transfer (EFT). (2) All-inclusive firm fixed price for Secure Data Exchange and Clearinghouse Services to cover all facilities in Scope of Work. Period is one year from beginning of services, with an option of up to 4 years. Offerors shall complete and submit to include pricing for all 5 years. (3) At a minimum, two (2) relevant past performance references for the most recently performed contracts in which similar or the same services were performed by the offeror. References shall include the agency or customer name and address, contract number (if applicable), contract period of performance, contract description of services, type and quantity of personnel provided dollar value of contract, and point of contact information (name, phone, facsimile, Internet/e-mail address). Offerors shall be evaluated on performance under the submitted relevant past performances. Offers who have no record of past performance shall be assigned a neutral past performance rating and shall not be penalized if they do not have a past performance record. This request for capability information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. The Oklahoma Area Indian Health Services has a requirement for Secure Data Exchange and Clearinghouse Services for the Oklahoma Area Indian Health Services. The Indian Health Services intends to award a firm-fixed price award for this requirement. If your organization has the potential capacity to perform these contract services, please provide the following information: 1) Organization name, address, email address, website address, telephone number, and size and type of ownership for the organization; and 2) Tailored capability statements addressing the particulars of this effort and Statement of Work, with appropriate documentation supporting claims of organizational and staff capability.
