Premium Service Agreement- Multiple Components Maintenance agreement for mobile Robot: • One (1) Preventative Maintenance Visit performed annuallywithin the 12 month contract period • On-Site repairs (labor, travel and parts) Accessories and consumables not included • Complimentary consultations and discounted rates for Integration Support Services and Systems Relocations Services • 6 Hour Phone Response, excluding evenings, weekends and holidays • 24-72 hour On-Site Response Time, excluding evenings, weekends and holidays • Annual new user Robot Training for up to two (2) operators • Includes device driver upgrades and updates • Unlimited phone and technical support • Up to 4 device driver exchanges within the 12-month period. • Hot-fix bug corrections Level of Effort: PM Frequency: 1 per year Government Responsibilities: NCATS will coordinate with vendor in regards to scheduling preventative maintenance visits. NCATS will provide access to the facility and access to the units being serviced. There are no other government responsibilities. Delivery or Deliverables: The vendor will perform preventative maintenance service and necessary repair service and suggest further maintenance as needed. Detailed reports of service performed will be provided. The period of performance is one year from the date P.O. is established Service to be billed quartlerly in arrears. Reporting Requirements: Reports will be emailed to NCATS
