Solicitation Number: 2021-53804 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2021-53804 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. FULL and OPEN COMPETITION The NAICS Code for this synopsis/solicitation is 213113, Size Standard 500 employees. Solicitation/ Purchase Request issued as a Request for Quotes (RFQ) number 2021-53804. This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2021-03, Effective 14 Jan 2021. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. The Government contemplates award of a firm-fixed price contract resulting from this solicitation for the following Contract Line Item Number (CLIN): CLIN 0001: PA-VA man hours 1330 Hours $ Unit Price Total Price $ POP: 7/1/2021 – 9/30/2021 This performance work statement establishes the requirements for a qualified Contractor to provide mine support services for the underground coal mines located on the NIOSH Pittsburgh site. The Contractor shall perform all related services necessary to carry out the provisions of this contract, including quality control, financial control, maintenance of accurate and complete records, files, and libraries of documents to include Federal, state, and local regulations, codes, laws, technical manuals, and manufacturers’ instructions and recommendations. The Contractor shall prepare and provide required reports, compile historical data, perform administrative work, and submit necessary information as specified in this contract. Such functions are in direct support of the research mission of CDC/NIOSH. Your quote shall include the following: The quote format is at the discretion of the offeror.
