CDC awarded contract number 75D30123F16539 to STC Health LLC, 411 S 1st St., Phoenix, AZ 85004 in the amount of $8,969,815.51. This notice makes available the Limited Sources Justification that applies to this contract. This contract was issued under the authority of FAR 8.405-6(a)(1)(i)(C), in the interest of economy end efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order or BPA must not have been previously issued under sole-source or limited sources procedures. This contract provides immunization information technology (IT) software and services for the Louisiana Immunization Network support. The period of performance is July 17, 2023 through July 16, 2026. The attached Limited Sources Justification is being made available within 14 days of contract award in accordance with FAR 8.405-6(a)(2) and will remain active for 30 days.
