Items to be quoted or equivalent: 1 EA DR-M2/1550/Refractometer Please also return the following along with your quote: 1. AMS-Form-009: Representations and Certifications – Procurement Specific. Due to the value of this contract, DOE requires Business Representations and Certifications be completed for all procurements exceeding $10,000. Please reference Solicitation Number VS393638 on the form and all correspondence. Please complete and return all twelve (12) pages of the document. 2. Published Price list: showing the items on your quote for price justification or a prior customers invoice for the same/similar items (personal info whited out). 3. Insurance: **IF APPLICABLE** Upon award, in accordance with Article 6 of the attached “BSA Supplemental Terms and Conditions for Work by Contractors On Site at Brookhaven National Laboratory”. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization. Equivalent Guidelines: The parts or model numbers are listed for reference purposes only. Any alternate products being offered shall have equivalent, or greater, performance parameters to those specified. Suppliers that are offering alternate products shall list the manufacturer, model, and part numbers for the product. When quoting an alternate product, please provide sufficient documentation to permit thorough evaluation of the equivalency of your offer to the product specified. BSA reserves the right to determine equivalency. 6.
