SOURCES SOUGHT SYNOPSIS / REQUEST FOR INFORMATION Solicitation Number: TBD Date: July 6, 2023 No solicitation is available at this time. This RFI is released for the purpose of conducting market research only. This announcement shall NOT be construed as a commitment by Brookhaven Science Associates LLC (BSA) to award a Subcontract. Brookhaven National Laboratory (BNL) reserves the right to use any and all information submitted by, or obtained from, an interested party in any manner BNL determines is appropriate, including, but not limited to, the creation of a competitive solicitation. An interested party shall NOT include any business confidential and/or proprietary information in its response, which will only be used internally for feasibility and market research and not shared with any external sources. BNL will NOT pay for any information that is provided in response to this announcement nor will it compensate any respondents for the development of such information. The purpose of this RFI is to gain market input to support our market research and obtain capabilities from potential Subcontractors. All official RFI responses with answers regarding the below questions shall be submitted via e-mail to the BNL Point of Contact, Sandra Moran at Sandra.Moran@bnl.gov, no later than 5:00 p.m. EST on, July 14, 2023. BNL personnel may contact firms responding to this announcement to clarify a responder’s capabilities or other matters as part of this market research process. Background: BNL is a Department of Energy (DOE) National Laboratory multidisciplinary research facility, located on Long Island, New York. The Occupation Medicine Clinic (OMC) is an on-site organization of dedicated BNL health care professionals. OMC's mission is to support the health of BNL employees in order to assure a healthy, safe and productive workforce for the benefit of BNL and of the nation. We achieve this mission through an emphasis on effective prevention and optimal management of work-related health problems, in order to promote a healthy workforce. Need: BNL requests any potential vendor to be capable of administering a stress test at a designated medical office, upon receipt of physician prescription. Results shall be delivered directly to OMC within four days of performing the tests. Potential vendors must be within a 100-mile radius of the BNL site. Please see attached DRAFT Statement of Work (SOW) for additional needs. Period of Performance: BNL is considering a period of performance of one (1) base year of twelve (12) months with the option to extend for up to an additional four (4) twelve (12) month terms. Response Requirement: All interested parties submit an electronic response. They shall be no less than 11-point font, 10 pages or less, and 20 Megabytes or less in PDF format. 1. Provide the name of your Firm (firms if teaming), point of contact, phone number, address of firm, Unique Entity ID (UEI), and/or CAGE code used over the past five years for the prime and each contemplated teaming partner. Small business teams shall identify socio-economic status as one or more of the following under NAICS 622110 – “General Medical and Surgical Hospitals” Size Standard $47 Million: 1) small business; 2) 8(a) business (including graduation date); 3) HUBZone small business; 4) small, disadvantaged business; 5) woman-owned small business; 6) service-disabled, veteran-owned small business; or 7) large business. 2. Identify any challenges or risks in meeting the requirements based on the information provided herein and as attached hereto this RFI or any issues that BNL should consider when developing the solicitation. 3.
