Pursuant to its obligations to the United States Government (represented by the Department of Energy under Contract No. DE-AC02-07CH11359), Fermi Research Alliance, LLC, herein after referred to as “Fermi National Accelerator Laboratory” or “Fermilab”, requests your written proposal for Safety Shoe Footwear Mobile Services in accordance with all attachments contained within the Fermilab Procurement Department Request for Proposal. Supplier shall provide ANSI Class 75 safety toe footwear per Safety Shoe Footwear Statement of Work (Schedule “A”), according to Safety Shoe Material Specifications and Size Range (Schedule “B”) and compliant with FESHM 4130: Personal Protective Equipment (Exhibit “C”). Services shall include labor, material, equipment, transportation and supervision. A fixed price IDIQ subcontract shall be issued by FRA. Services will be provided on a 2-week cycle. The award will be made based on the lowest price. The North American Industry Classification System (NAICS) code is 316210.
