Request for Quotation for a BioLogic Potentiostat with components or it’s equivalent as described below: BioLogic Potentiostat which must include and meet the full configuration and specifications of the following: Items to be quoted or its Equivalent: Qty. 1 Model VSP300-CHAS. VSP-300 chassis – 6 slots - NO channel - including calibration board & EC-Lab software package. Qty. 4 Model VSP300-01 Potentiostat/galvanostat board for SP-300/VSP-300/VMP-300 (without cell cable) – uses one slot - +/-12 V +/-500 mA. Qty. 2 Model VSP300/Z-01 Potentiostat/galvanostat board with EIS(/Z) option for SP-300/VSP-300/VMP-300 (without cell cable) – uses one slot – +/-12 V +/-500 mA. Qty. 6 Model VSP300-CB Standard cell cable with electrometer for SP-300/VSP-300/VMP-300 potentiostat board (1.75 meter). NRTL-CERT. Safety certification by a nationally recognized testing laboratory for the complete system. QUALIFIERS: 1. Offerers must provide a commercially available product meeting the performance and specifications of the item identified herein. 2. Will the Offerer provide all of the items as listed above with specified manufacturer and item numbers? If no, Offerer shall submit a detailed specification and test report that illustrates equivalent requirements. 3. Does the Offerer have previous experience manufacturing, assembling, and testing Potentiostats of similar complexity for the items identified above? If yes, the Offerer shall provide references for at least three (3) successfully manufactured, assembled, tested and delivered 3 Potentiostats of similar complexity. The references shall include the name of the organization, contacts, phone numbers, and e-mail addresses. Please also return the following along with your quote: 1. AMS-Form-009: Representations and Certifications – Procurement Specific. Due to the value of this contract, DOE requires Business Representations and Certifications be completed for all procurements exceeding $10,000. Please reference Solicitation Number 390732 on the form and all correspondence. Please complete and return all twelve (12) pages of the document. 2. Published Price list: showing the items on your quote for price justification or a prior customers invoice for the same/similar items (personal info whited out). 3. Insurance: **IF APPLICABLE** Upon award, in accordance with Article 6 of the attached “BSA Supplemental Terms and Conditions for Work by Contractors on Site at Brookhaven National Laboratory”. Prior to coming on site to perform work, the certificate must remain in effect for the duration of the subject contract and must be filed with BNL Procurement & Property Management Division. 4. System for Award Management (SAM): In order to support DOE reporting requirements, BSA requires all its vendors/contractors to be registered in the U. S. Government supplier database entitled System For Award Management (SAM) with their Dun & Bradstreet number (Duns#). Registration can be performed online at www.sam.gov. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization. Please note that there is no charge for SAM registration. When registering, you are strongly urged to copy and paste www.sam.gov onto your browser address rather than utilizing your search engine. Equivalent Guidelines: The parts or model numbers are listed for reference purposes only. Any alternate products being offered shall have equivalent, or greater, performance parameters to those specified. Suppliers that are offering alternate products shall list the manufacturer, model, and part numbers for the product. When quoting an alternate product, please provide sufficient documentation to permit thorough evaluation of the equivalency of your offer to the product specified. BSA reserves the right to determine equivalency. 6. Basis of Award: BSA will make award to the responsive and responsible offeror who submits the lowest total price and who meets all of BSA’s technical requirements, as stated herein. 7.
