Insurance: **IF APPLICABLE** Upon award, in accordance with Article 6 (shown below) of the attached “BSA Supplemental Terms and Conditions for Work by Contractors On Site at Brookhaven National Laboratory”. Prior to coming on site to perform work, the certificate must remain in effect for the duration of the subject contract and must be filed with BNL Procurement & Property Management Division. Please have certificate sent with the following in highlight: The Contractor shall, at no cost to BSA, maintain policies providing the following insurance protection, which insurance shall apply to all operations of the Contractor hereunder and employees of the Contractor engaged therein. Unless waived in writing by BSA’s PPM Division, the Contractor shall also provide an endorsement to its liability policies either naming Brookhaven Science Associates, LLC and the U.S. Government as additional insureds or insuring the Contractor’s obligations under the paragraph hereof entitled “Independent Contractor; Hold Harmless.” Worker’s Compensation – Coverage as provided in the Worker’s Compensation Law of the State of New York having jurisdiction, including occupational disease coverage for limits of $1,000,000 per person in any one case and additional Employees Liability of $1,000,000 per occurrence. General Liability – Insurance with limits of $1,000,000/$2,000,000 for bodily injury liability and $100,000 for property damage liability in the comprehensive liability form. Automobile Liability – Insurance with limits of $250,000/500,000 for bodily injury liability and $50,000 for property damage liability in the comprehensive policy form. The Contractor may purchase such additional or other insurance protection, as it may deem necessary, at its own expense. The Contractor shall furnish BSA’s PPM Division a certificate of insurance to show compliance with paragraph (a) above. The insurance certificate shall be submitted within fourteen (14) days of award and prior to issuance of a Notice to Proceed. The Contractor shall also ensure that such certificate states that the insurance carrier(s) will give BSA 30 days, prior written notice if there is any cancellation or material change in such policies. The Contractor shall also ensure that such certificates are kept up to date during the period of Contract performance. System For Award Management (SAM): In order to support DOE reporting requirements, BSA requires all its vendors/contractors to be registered in the U. S. Government supplier database entitled System For Award Management (SAM) with their Dun & Bradstreet number (Duns#). Registration can be performed on line at www.sam.gov. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization. Please note that there is no charge for SAM registration. When registering, you are strongly urged to copy and paste www.sam.gov onto your browser address rather than utilizing your search engine. Equivalent Guidelines The parts or model numbers are listed for reference purposes only. Any alternate products being offered shall have equivalent, or greater, performance parameters to those specified. Suppliers that are offering alternate products shall list the manufacturer, model, and part numbers for the product. When quoting an alternate product, please provide sufficient documentation to permit thorough evaluation of the equivalency of your offer to the product specified.
