Requirements for this solicitation are outlined in the attached RFQ Document. Quote package shall include the below: Price Quote: The Offeror shall complete and return the attached Quotation Pricing Sheet. Pricing shall remain firm for sixty (60) days to allow time for review, selection, and Award. Prices shall be quoted in U.S. Dollars. This requirement is subject to FAR Part 25, Buy American Act. Please provide the place of manufacturer on the Quotation Pricing Sheet. The Offeror shall provide the requested information for listed Technical Qualifiers as detailed in section 2.1. Freight terms quoted shall be FOB Destination, Freight Prepaid as detailed on Page 1 of RFQ Document. Other Submittals: SAM Unique Entity Identification (UEI) Number within attached Quotation Pricing Sheet, as well as affirm Representations and Certifications and size status are accurate and up to date in SAM. * Foreign Offerors are not required to be registered in SAM; however, must fill out the attached AMS-Form-050 (Annual Representations and Certifications) if they are not. The attached AMS-Form-050 (Annual Representations and Certifications) is only required for Offerors that are not currently active and registered in SAM. A current copy (less than 12 months prior to the date of solicitation) of AMS-Form-050 (Annual Representations and Certifications) must be on file with BSA’s Procurement and Property Management (PPM) Division before an Award can be placed. The successful Offeror will be required to complete new vendor forms, if applicable.
