INTRODUCTION Brookhaven Science Associates, LLC (herein known as “BSA”), operator of Brookhaven National Laboratory (BNL) under a Prime Contract with the U.S. Department of Energy (DOE) herewith solicits your quote for Health Physics Support as outlined in Enclosure 1, Draft Contract No. 442492. SCOPE OF WORK The Offeror shall provide technical staffing services in the areas of Health Physics, Radiation Safety, and Accelerator Safety. Labor Categories include; Decontamination and Decommissioning Helper, D&D Technician I, D&D Technician II, D&D Technician III, Radiological Control Technician I, RCT I (Junior Technician), Radiological Control Technician II, RCT II (Senior Technician), Lead Radiological Control Technician, Technical Supervisor, D&D Supervisor, and Radiological Engineer all in strict accordance with Enclosure 1, Draft Contract No. 442492. FORMS OF QUOTATION AND MANNER OF SUBMISSION A complete quotation, including the signed Quotation Pricing Sheet, must be received by BSA no later than the date and time stated herein. Any offer received after this time and date may not be considered. Award is expected to be made on or about August 1, 2024. Quotations will be opened in private. Quotations, including their withdrawal or modification, received after the closing date shall be considered late. Such responses will only be considered for award if it is in BSA’s best interest to do so. Electronic submissions will be accepted by: Email - submission must be to chornbeck@bnl.gov. The subject line shall state “RFQ 442492, Health Physics Support”. PRICING AND SUPPORTING DOCUMENTATION Pricing Quotation and Supporting Documentation. The offeror shall complete and return the Quotation Pricing Sheet attached as Enclosure 2. Prices shall be quoted in U.S. dollars, FOB, Brookhaven National Laboratory, Upton, Long Island, New York (freight included) including any Import Duties, customs and associated costs, if applicable. System For Award Management (SAM): In order to support DOE reporting requirements, BSA requires all its vendors/contractors to be registered in the U. S. Government supplier database entitled System For Award Management (SAM). Registration can be performed on line at www.sam.gov. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization. Please note that there is no charge for SAM registration. When registering, you are strongly urged to copy and paste www.sam.gov onto your browser address rather than utilizing your search engine. By signing the attached Quotation Pricing Sheet, AMS Form-043, it shall be assumed that your firm is registered in SAM. 4.3 The intended awardee may be required to fill out and return a completed W-9 form and a Bank Form prior to any award being made. 4.4 The intended awardee shall be required to fill out and return a completed Organizational Conflicts of Interest Disclosure form prior to any award being made. 4.5 Subcontracting Plan: Large Businesses submitting a quote must comply with FAR 52.219-8, Utilization of Small Business Concerns and shall submit a Subcontracting Plan with their quote submission. BASIS OF AWARD BSA will make award to the responsive and responsible offeror who submits the lowest total price (to include options) and who meets all of BSA’s qualifying requirements, as stated herein. Qualifier(s): 1. Offeror must have ten (10) years or more experience providing professional health physics and 10CFR835 compliance support to Department of Energy national laboratory radiological protection programs. The offeror shall provide reference documentation demonstrating 10 years or more of support services provided to the Department of Energy’s National Laboratory radiological protection programs. The references shall include the name of the organizations, services provided, contacts, phone numbers, and e-mail addresses. 2. Offeror must demonstrate technical and professional competence in health physics, radiation protection, and accelerator safety health physics. The offeror shall provide evidence of their staff qualifications and experience that show a combination of Certified Health Physicists (CHPs) and/or professionals with advanced degrees (i.e., M.S, Ph. D) in health physics, nuclear engineering, or a closely related field. 3. Offeror must have five (5) years or more experience providing DOE Order 420.2, Safety of Accelerators, compliant services. The offeror shall provide reference documentation demonstrating 5 years or more of DOE O 420.2 compliant support services provided to Department of Energy national laboratory radiological protection programs and/or accelerator safety programs. The resultant Labor Hour contract shall be subject to the contract number of the Federal Supply Schedule Contract between the General Services Administration (GSA) and the Contractor that is pertinent to this procurement. In the event of any inconsistency between the terms and conditions of this contract and those of your Federal Supply Schedule Contract, the latter will govern.
