Request for Blanket Purchase Agreeement The contractor shall provide all material, and labor necessary to provide new/repair services on the various building overhead doors, pedestrian doors, automatic doors, security doors, dock levelers/restraints, gate operators, etc. as requested, at Argonne National Laboratory (ANL). Contractor must provide parts to the Laboratory when requested by a requisition/call for specific maintenance repairs to be completed by ANL. The contractor must have: A qualified crane/bucket truck operator with proof of training and current medical surveillance and one person must have signal person training. Fall Protection Competent Person Training LOTO training including NFPA 70E Training. OSHA (30) Hour Trained Individual Elevated work platform/boom lift training Fork-truck operator training including attachments and signal person training, current medical surveillance for operator. High Risk JSA, all required training documentation, and Contractor Safety Orientation required before beginning work. 10 Years (as a company) performing Overhead Door, pedestrian door, automatic door and other specified maintenance and repairs. Open face cord wheels for all welding reels (100ft of welding leads) The contractor shall estimate all door repairs or upgrades and submit a written proposal to the ANL/PMO/Rapid Response Technical Representative for this BPA service contract. Proposals submitted shall contain the following information: Building and door number and/or location. Detailed description of the repair or upgrade/replacement required. Labor hours breakdown & billing rates as per this agreement Material costs breakdown Equipment breakdown Estimated time for receiving material. The technical representative will reject proposals not submitted in this manner. All work covered under this agreement shall be authorized by the technical representative or listed BPA alternate. Failure of the contractor to obtain such approval shall result in the non-reimbursement of costs incurred by the contractor. Please reach out to coordinate an on-site visit, if desired.
