This contract consists of two (2) Medical Assistants/Care Coordinators(12100) and (4) Medical Records Technicians(12190). Place of Performance: Camp Niantic, Building 804 located at 38 Smith St., Niantic, Connecticut 06357 Questions: In order to provide timely responses to all questions, no questions/RFIs will be accepted after 9:00 AM EST on 15 August, 2024. Submittal of Offers: Offers shall be received not later than 9:00 am EST on August 28, 2024. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). Quotes will only be accepted via email by James Turcotte at james.e.turcotte3.civ@army.mil. BASIS OF AWARD- EVALUATION I. Basis for Contract Award – Evaluation of Offers-Simplified Acquisition Procedures (a) In accordance with FAR 13.106 and FAR subpart 13.5, the Government will award a contract resulting from this solicitation to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following shall be used to evaluate offers: Experience Past performance; and Price (b) Options. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR clause 52.217-8, Option to Extend Services, if included in this solicitation. Options under FAR clause 52.217-8 will be evaluated by using the prices offered for the last option period to determine the price for a 6-month extension, which will be added to the base and other option years to arrive at a total evaluated price. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier’s quotation does not establish a contract. A written notice of award or purchase order, emailed or otherwise furnished to the successful quoter is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer. II. Quotation Evaluation: The evaluation process will be accomplished as follows: General: The government intends to make award to the responsible quoter who represents the best value to the Government based on (1) Relevant Experience; (2) Past Performance that is recent, relevant and has a satisfactory or better confidence rating and (3) Price fair and reasonableness. PRICE: Quoters are required to provide a breakdown of pricing and narrative of the following: Number of Medical Assistants/Care Coordinators and Medical Records Technicians being quoted for each year for base and options. IF quoting a staffing plan with fewer full-time employee’s than shown in PWS 1.1 that is fine; however, a detailed explanation of how you intend to get the work done with fewer full-time employees shall be provided. This needs to clearly explain your innovative approach and how the requirements of the PWS will be met. If detailed explanation not provided, the quote will be considered non-compliant and will not be considered. This is only needed IF quoting fewer full-time employees than shown. Price will be evaluated for fairness and reasonableness in accordance with FAR 13.106-3(a). III. FACTORS TO BE EVALUATED: FACTOR 1: Experience: We will evaluate what work you did, when and where you did it, whom you did it for, and what methods you presumably used. It is important to understand the difference between a quoter’s experience and its past performance – quoters may be considered unacceptable based on its lack of relevant experience. Relevant experience is adequate experience consisting of at least three years performing same/similar case management projects that are comparable to the work requirements of this solicitation. Experience will be rated as Relevant, Somewhat Relevant, and Not Relevant. Standard of Proof: Submit the below information, to provide evidence of a minimum of three years relevant experience. Number of years the Quoter has been in business and adequate evidence. Experience showing a minimum of three years performing same or similar case management projects. Includes scope, magnitude of effort, and complexities as shown below. A list of present relevant commitments, including the dollar value. Identify all relevant projects that are substantially complete or have been completed within the past three years of the solicitation issue date. Experience Evaluation Ratings: The following table will be used to evaluate experience. Your past performance is relevant to this competition when you have done recent work that has confronted you with the same kinds of challenges that you would reasonably expect to encounter during performance of the contract described by the PWS. We will assess your past performance on the basis of recency, relevancy, and quality. Past performance information may be obtained from any sources available to the Government (CPARS)(SAM). First, we will evaluate recency. For purposes of this evaluation, recency is defined as demonstrated experience (active or completed efforts) performed within the past three (3) years from the issuance date of this solicitation. Second, we will evaluate recent contracts for relevancy as described below. Third, we will evaluate relevant contracts for overall quality. The evaluation of the Quoter’s (including partnerships and joint venture) present/past performance WILL NOT include the present/past performance of any subcontractors, predecessor companies, affiliates, other divisions or corporate management even though they may perform major or critical aspects of this requirement. Only the Quoter's (includes partnerships and joint ventures) past performance will be evaluated. Past Performance Evaluation Ratings: As a result of the recency and relevancy assessments of current and prior contracts, a relevancy rating described below will be assigned within the past performance evaluation. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol. (f) WAWF payment instructions. The Contractor shall use the following information when submitting payment requests and receiving reports in WAWF for this contract or task or delivery order: (1) Document type. The Contractor shall submit payment requests using the following document type(s): (i) For cost-type line items, including labor-hour or time-and-materials, submit a cost voucher. (ii) For fixed price line items— (A) That require shipment of a deliverable, submit the invoice and receiving report specified by the Contracting Officer. (B) For services that do not require shipment of a deliverable, submit either the Invoice 2in1, which meets the requirements for the invoice and receiving report, or the applicable invoice and receiving report, as specified by the Contracting Officer.
