The services will be of a nature and scope described in detail in the Performance Work Statement. The contract Health Care Provider (CSP) will work in conjunction with other appointment clerk, professionals, and non-contract personnel. The contactor shall provide the service of Appointment Clerks to be performed at Blanchfield Army Community Hospital and other satellite clinics, Ft Campbell, KY 42223 as directed by the Chief, Clinical Support division. The Contractor shall perform services in accordance with the ethical professional standards of the health care industry. See attached SF 1449 for details. Amendment 0001 issued to extend the due date to allow more time for Government to respond to Contractor questions. Amendment 0002 is to add a mandatory clause by reference. Amendment 0003 issued to extend the due date to allow more time for Government to respond to Contractor questions. The requirement for Medical Appointment Clerk Servcies. This requirement is set aside 100% for small businesses in the Small Business Administration 8(a) Program. This is a non-personal service contract to provide health care support for the Blanchfield Army Community Hospital, Fort Campbell, KY. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government in accordance with all terms and conditions in the contract. Except as otherwise limited herein, the Contractor may use or disclose Protected Health Information on behalf of the Government for treatment, payment, or healthcare operations purposes, in accordance with the specific use and disclosure provisions.
