PERFORMANCE WORK STATEMENT FAMILY NURSE PRACTITIONER 8TH MEDICAL GROUP KUNSAN AB, REPUBLIC OF KOREA 1 QUALIFICATIONS. 1.1. Must be knowledgeable of clinical application of principles and practices related to the nursing process or healthcare management. Requires prior experience in case management, discharge planning, utilization management, process improvement, and personnel management. Must be able to review and understand a wide range of medical disorders, conditions, and disease processes for the adult patient, to include in-person and via-telephone assessments, evaluations, and diagnoses. In addition, must be able to educate and apply preventive health and disease management principles with a primary focus on treatment and prevention protocols. Must have a working knowledge of commonly used pharmacological agents used in the adult population. Knowledge of computer applications and software to include Microsoft Office programs, MS Outlook (e-mail), internet, Excel, and PowerPoint. Knowledge of electronic healthcare record documentation in Composite Health Care Systems (CHCS) and Armed Forces Health Longitudinal Technology Application (AHLTA) is required. Must successfully complete all facility orientation requirements as a clinical nurse. 1.2. Must be skilled in analyzing and defining healthcare needs of a defined population using standardized assessment tools. Able to meet administrative requirements for proper documentation of patient's condition including disease progress, education, and follow-up care. Must be able to use problem-solving techniques to articulate medical requirements to patients, family members, care givers and medical as well as non-medical staff in a professional and courteous way. 1.3. EDUCATION: Must have successfully completed all education and training requirements leading to graduation from an accredited national, state, or military equivalent school for Family Nurse Practitioner (FNP). FNP board certification or equivalent is mandatory. 1.4. LICENSE: Must have and maintain throughout course of contract, a current, active, valid, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice. Must maintain requirements mandated by credentialing agency, to include continuing education requirements. Shall have, and maintain, current certification in Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) as certified by the American Red Cross. Have a minimum of at least two years of primary care experience treating patients of all ages for routine, urgent and emergent diseases and injuries and no more than twelve (12) months since the last work assignment. Contract HCPs determined by the MTF to be involved in the delivery of healthcare to children under the age of 18 on a frequent and regular basis must meet the requirements of the Crime Prevention Act of 1990. If required, contract health care providers who write prescriptions for controlled substances must have a current Drug Enforcement Agency (DEA) certificate or Controlled Drug Substance number as appropriate. All contract HCPs shall be able to read, write, and speak English well enough to effectively communicate with patients, other Health Care Providers and employees of the MTF. All contract HCPs shall be required to meet Continuing Medical Education (CME) requirements. The Contractor shall ensure contract HCPs have received Occupational Safety and Health Administration (OSHA) required training for the position in which they will perform, and that training is maintained current. Documentation of such training shall be provided to the COR. Contractor(s) will (1) actively participate in the site's safety and health management system and comply with OSHA standards; and (2) report all workplace hazards, accidents and near-miss events to the supervisor and Medical Department Activity Safety Manager. • Certification/Recertification: All certification required elsewhere in this performance 1.4. KNOWLEDGE: 1.4.1. Knowledge of professional care theories, principles, practices, disease process, and procedures. 1.4.2. Knowledge of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required. Knowledge of the administrative requirements for proper documentation of patient's condition including disease progress, acknowledgement of teaching, and follow up care. 1.4.3. Knowledge of computer operations and proficiency in the use of basic word processing, data entry, and automated medical records. 1.4.4. Skill in setting up, operating and monitoring specialized medical equipment such as cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, and other unit specific equipment. 1.5. EXPERIENCE: 1.5.1. A minimum of 2 years of experience as FNP in English-speaking setting is required, with experience in both acute and chronic care settings. 1.5.2. Exposure to and fluency in English language is required. Must be able to write, speak, and understand English language. Fluency in Hangul is desired but not required. 1.5.3. Experience performing care and treatment of patients, operating and maintaining therapeutic equipment, properly administering medications, conducting paraprofessional portions of clinical visits to include telephone consultations, and assisting in medical treatment of patients in accordance with established procedures and protocols. 1.6. Language Requirement. Contractor shall read, understand, speak, and write English. Korean contractor shall read, understand, speak, and write both English and Korean well enough to effectively communicate with patients, coworkers, and healthcare providers without language-related interruptions. A foreign national candidate must have a TOEIC score above 550. 2. TYPE OF WORK. The duties include but are not limited to the following: 2.1. Provide a full range of FNP services in accordance with privileges granted by the Commanding Officer (e.g., supervise and provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; provide immunizations; diagnose, treat, and counsel patients as indicated). 2.2. Treat patients with common acute conditions, illnesses, or minor trauma within accepted protocols, Nurse Practice Acts, and/or in collaboration with a physician. 2.3. Collaborate with the physician in the health care of patients with acute and chronic illnesses. 2.4. Request consultation or referral with appropriate physicians, clinics, or other health resources as indicated. 2.5. Order diagnostic tests as applicable. 2.6. Prescribe and dispense medications as delineated by the Pharmacy and Therapeutics Committee. 2.7. Promote preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling. 2.8. Coordinates patient care through a continuum and facilitates the achievement of optimal outcomes in relation to care, quality and cost effectiveness 2.9. Ensures compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment facility. Provides care within ethical and legal boundaries. 2.10. Assesses patients. 2.11. Ability to effectively communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. Effectively uses appropriate communication format in addressing professional issues. Collaborates with off-base medical facilities as needed for sake of patient care. 2.12. Excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements. 2.13. Ability to recognize adverse signs and symptoms and react quickly in emergency situations. 2.14. Completes orientation and competency verification programs in accordance with (IAW) unit guidelines. 2.15. Participates in the orientation of newly assigned personnel, as appropriate. 2.16. Serves and participates in committees, functions and other meetings as directed. Provides relevant and timely information to these groups, and assists with decision-making and process improvement. Participates in customer service initiatives and medical readiness activities designed to enhance health services. 2.17. Ensures a safe work environment, employee safe work habits and patient safety IAW regulatory agencies, infection control policies, and process improvement initiatives. 2.18. In addition to the education, certification and experience listed in the basic position description above, the 8th Medical Group (Kunsan AB) requires the Family Nurse Practitioner to perform deployment related health assessment (DHRA), annual mental health assessment (MHA), non-aeromedical preventive health assessment (PHA), Base Operational Medicine Clinic (BOMC) support, and primary care duties that comply with applicable Department of Defense and Air Force policy and guidance, to include required standards of care and clinical practice guidelines. Additionally, the Family Nurse Practitioner must a have working knowledge of Microsoft Office applications and become proficient in the use of the following computer systems: Composite Health Care Systems (CHCS), Armed Forces Health Longitudinal Technology Application (AHLTA), Aeromedical Services Information Management System (ASIMS), and Preventive Health Assessment and Individual Medical Readiness (PIMR). 2.19. The FNP shall have sufficient specific training or practice experience in Family Medicine and shall be able to adequately assess, diagnose, stabilize and treat diseases and injuries to include, but not limited to, the following: Skin, Head, Ears, Eyes, Nose, Throat, Mouth, Teeth, Neck, Cardiovascular System, Pulmonary System, Gastrointestinal System, Lymphatic System, Musculoskeletal System, Genitourinary System, Reproductive System, Psychiatric Disease, Central Nervous System, Endocrine System, Peripheral Nervous System. The PA FNP shall be able to recognize those conditions beyond his/her level of training and/or experience and consult a physician in an appropriate and timely manner. 2.20. The FNP performing services under this contract shall have sufficient training and experience to be able to adequately perform credentialed medical procedures to include, but not limited to the following: ? Administer intravenous, intramuscular and subcutaneous medications ? Anoscopy ? Arthrocentesis (excludes hip) ? Burn management ? Cultures (throat/wound) ? Ear irrigation ? Eye irrigation ? Foreign Body Removal (rectal, extremity and uncomplicated ear/nose) ? Incision and drainage of abscesses & thrombosed hemorrhoids ? Ingrown toenail removal initial interpretation of radiographs ? Initial management of, envenomations and exposure to hazardous materials ? Initial management of orthopedic extremity injuries, uncomplicated (splints) ? Interpret laboratory results; Laceration repair, complex (excludes flexor tendon repair and repair of neurovascular damage), Laceration repair (simple) ? Local Anesthesia (topical/ infiltration) and Regional Anesthesia (digits) ? Nebulizer treatment; Ocular tonometry, fluorescein and slip lamp examination ? Perform venous puncture for laboratory studies; Secure and maintain an adequate airway ? Secure and maintain adequate intravenous access by peripheral routes or Suture removal ? Wound care (evaluate and direct management) ? Collect and record, in proper format, medical and social historical data appropriate to the patient's condition ? Perform a general physical examination. Record the findings in proper format and determine medical qualification for various purposes ? Become familiar with the hospital formulary and prescribe pharmaceuticals according to the drugs listed. The Pharmacy Service will provide instruction to all prescribing providers on substitutions of generic drugs ? Follow the established operating procedures of the MTF and clinics where services are provided. Providers shall participate in the quality control activities of the MTF to include the quality control activities established for each department or clinic the provider works ? Performs other duties as assigned ? The Contractor HCP shall participate in administrative or clinical duties as requested or needed to ensure sustainment of normal clinical operations and ensure compliance with Joint Commission (JC) standards 3. HOURS OF OPERATION. The following is a detailed description of the duty hours, leave policy, facility closure policy, and holiday policy. 3.1. The contractor shall not exceed 80 hours in a two-week period. The contractor will only be paid for actual hours worked. The medical facility is open from 0800 to 1700 hours, Monday through Friday. 3.2. The following is a list of legal federal holidays. Contract personnel may be required to work on legal holidays as determined by the COR: New Year's Day, January 1st Martin Luther King's Birthday, 3rd Monday in January President's Day, 3rd Monday in February Memorial Day, Last Monday in May Independence Day, July 4th Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veteran's Day, November 11th Thanksgiving Day, 4th Thursday in November Christmas Day, December 25th 3.3. Contract personnel shall report to work as scheduled during base exercises. Contract personnel shall also report to work during emergencies, disasters, or during adverse weather conditions. Contract services are still required during these situations. However, if transit to work is not possible, personnel are to notify COR and immediate supervisor. 3.4. During anticipated closure of the facility due to severe weather, military emergencies, scheduled "down-days," or other occasions, the contractor shall not invoice the government for services not performed and the Government will not be liable for any such closures. 3.5. The contractor shall not provide on-call/after-hours or overtime duties. 3.6. Unscheduled absences due to illness or emergency must be coordinated with the COR and immediate supervisor as early as possible. 3.7. Scheduled absences shall be coordinated with both the COR and immediate supervisor no less than 30 days prior to anticipated leave. 3.8. The contractor is responsible for submitting an accurate invoice. 4. ORIENTATION AND TRAINING. 4.1. When commencing work under this contract, the contract personnel shall obtain in-processing checklist from the COR or designated office. All contract personnel are required to complete the applicable items on the checklist and turn it in to the designated office. Upon completion of the duty assignment, the contract personnel need to complete an out-processing checklist and return it to the designated office. 4.2. The contract personnel shall be provided with a government paid orientation to familiarize the contract personnel with the policies and procedures of the medical facility, assigned department, and specific unit. This shall be completed within 30 days of starting the contract performance. 4.3. The contractor shall ensure the contract personnel complete any mandatory training requirements in accordance with medical facility procedures. 4.4. Contract personnel who have any interaction with the MTF computer systems must receive training for the applicable systems. The COR will coordinate the necessary computer training and it will take place on-site and during normal duty hours. The training will be at no cost to the contractor. This includes but is not limited to CHCS and AHLTA. 5. CONDUCT. Contract personnel shall comply with facility policies regarding dress, appearance, good order, and discipline. Contract personnel shall abide by all regulations regarding the nature of limited privileged communication between patients and the contract personnel, who may be necessary for security and personal reliability programs. Additionally, they shall also abide by all regulations concerning the confidentiality of patient records, as embodied in federal statutes including the Privacy Act of 1974 and the Health Insurance Portability and Accountability Act of 1996. All medical records and reports will remain the property of the U.S. Government. 6. HEALTH REQUIREMENTS. 6.1. All contract personnel performing services under this contract shall comply with 8th Medical Group health and immunization requirements. The expense to comply with the health and immunization requirements shall be borne by the contractor at no additional cost to the Government. 6.2. No later than five (5) working days prior to commencement of work, certification shall be provided to the COR that contract Medical Care Manager has completed medical evaluation required, as above. This certification shall state the date on which the examination was completed, the name of the medical provider that performed the examination, and a statement concerning the physical health of the individual. The certification shall also contain the following statement: "(name of contract employee) is free from contagious diseases." 6.3 The 8th Medical Group will provide emergency health care for contract personnel for injuries occurring while on duty in the MTF. These services will be billed to the contractor at the current full reimbursement rate. 6.4. It is the responsibility of the contractor to notify the COR of a pregnant employee. MTF Employee Health Office will provide information concerning any work hazards in that area. The contractor is to notify their pregnant employee of any work hazards. The contractor will provide a replacement for any time required off for maternity leave. 7.0 Performance Requirements Summary (PRS) 7.1 Contractor performance will be measured against the Acceptable Quality Levels in the Performance Requirements Summary (Table C-1). FRED will assess performance of contractor based on periodic surveillance, Monthly Contractor Performance Assessment Report, and customer service feedback 1) Contract Deficiency Report issued. 2) Corrective action taken immediately after CDR issued. 3) Progress meetings by KO The contract personnel shall respect and maintain the basic rights of patients, demonstrating concern for personal dignity and human relationships. Acceptable: The provider will not receive any more than three valid customer complaints per year. Unacceptable: The provider receives more than three valid customer complaints per year. 3.8.6.4. FRED will assess performance of contractor based on customer service feedback 1) Contract Deficiency Report issued. 2) Corrective action taken immediately after CDR issued. 3) Progress meetings by KO The Contractor is responsible for submitting an accurate invoice.
