This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912TF-22-Q-0034, and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-04. The associated North American Industrial Classification System (NAICS) code for this procurement is 621511- Medical Laboratories, a small business size standard of $35 million. This requirement is 100% set-aside for small business, and only qualified vendors may submit quotes. The solicitation will start on the date this solicitation is posted and will end on 06 July 2022 at 5:00 PM EST. The Government intends to award a Firm-Fixed Price (FFP) contract. This requirement is for Medical Laboratory Testing and Analysis services for the New Hampshire Army National Guard (NHARNG). This requirement is for (1) base year, plus (4) option years. Services are required in accordance with (IAW) the schedule provided in the attached Performance Work Statement (PWS). The NH National Guard has a requirement for the following service(s): CLIN 0001: Medical Laboratory Services (BASE YEAR), 1 JOB. Description: All testing and related services as described in paragraphs 5.4 through 5.4.3 of the PWS Period of Performance: 01 August 2022 – 31 July 2023 CLIN 0002: Medical Laboratory Services (OPTION YEAR 1), 1 JOB. Description: All testing and related services as described in paragraphs 5.4 through 5.4.3 of the PWS Period of Performance: 01 August 2023 – 31 July 2024 CLIN 0003: Medical Laboratory Services (OPTION YEAR 2), 1 JOB. Description: All testing and related services as described in paragraphs 5.4 through 5.4.3 of the PWS Period of Performance: 01 August 2024 – 31 July 2025 CLIN 0004: Medical Laboratory Services (OPTION YEAR 3), 1 JOB. Description: All testing and related services as described in paragraphs 5.4 through 5.4.3 of the PWS. Period of Performance: 01 August 2025 – 31 July 2026 CLIN 0005: Medical Laboratory Services (OPTION YEAR 4), 1 JOB.
