The project requires a contractor to provide all supervision, personnel, equipment, supplies, and materials for pavement crack filling and seal coating on the Brookville Lake Dam Access Road. Work will be performed at 10064 Overlook Rd, Brookville, IN 47012, and must comply with attached specifications and applicable federal, state, and local laws and regulations. A site visit is recommended and can be scheduled by contacting Zach Jones; questions should be submitted through ProjNet, with other inquiries directed to Justin McPherson. Quotes must be submitted electronically through PIEE only; fax, telephone, mailed, or emailed submissions will not be accepted. Amendment 0001 revises the Price Breakout Schedule to clarify that the estimated 87,500 square feet of work includes the dam road, turnaround, and parking lot off the turnaround as shown in Exhibit D, with the total size unchanged, and offers must acknowledge all amendments and use the revised PBS.
