Amendment 0001: The Scope of Work has been replaced. Please review thoroughly. The proposal due date is not extended. The Contractor will provide professional services as a Registered Nurse not to exceed 800 hours for a one-year period as directed by the government nurse in the U.S. Army Corps of Engineers, Louisville District Occupational Health Unit, located in Louisville, Kentucky in the Ron Mazzoli Federal Building, Rm. 138. These medical services are for all government employees in the participating agencies and others in an emergency. The Contractor will work under the technical direction of the government nurse and under the administrative control of the Occupational Health and Safety Office, U.S. Army Corps of Engineers, Louisville District. Services will be performed on an as-needed basis for up to eight (8) hours per day. The hours are between 0630a and 1830p Eastern Time, Monday – Friday. Will observe Federal Holidays along with the day after Thanksgiving and the week between Christmas to New Year’s annually. Payment will be made on actual hours worked with no accumulation of sick leave, annual leave, or holiday compensation. Contractor must agree to be paid electronically and be responsible for all taxes, workman’s compensation, social security and professional liability and health insurance. The Government is obligated only to the extent of such requests as are actually made against this contract. The Government will give the Contractor a reasonable notice, except under extreme emergency conditions, prior to when service is required to be performed. If the Contractor cannot perform when notified under an emergency condition, services will be procured elsewhere. Quantities should not exceed 800 hours per year. The contract will be for the base period from 01 April 2023 through 31 March 2024 and include options to extend the period of services for four (4) one-year periods. Total contract amount is estimated.
