Occupational Medical Surveillance Examination Program SOURCES SOUGHT: The purpose of this sources sought is to gain knowledge of potential sources for this project including interested Small Business sources. This notice is for planning purposes only. All interested parties should respond. South Carolina Army National Guard may issue a solicitation and award of a contract to Occupational Medical Surveillance Examination Program subject to availability of funds. The Occupational Medical Surveillance Examination Program is designed to provide job-related Medical Surveillance Examinations and Occupational Health Services to the South Carolina Army National Guard full-time support (FTS) and/or temporary personnel who are, or could potentially be, exposed to health hazards in the work environment. Selected personnel will receive comprehensive exams while others will receive limited testing. The basic Physical should include the following services: • Medical Questionnaire • Blood Testing (CBC, CMP) • Urinalysis • Vital Signs • Height, Weight, and Waist-to-Height Ratio • Vision Screening • Physical Exam and Consultation • Board Certified Occupational Medicine Physician Overview • Record processing exams/clearances • Digital records emailed Additional Services as needed- • Audiometry • Spirometry • Chest X-Rays • B-Reader for ILO X-Ray • Kraus-Weber Assessment • Cadmium Profile • Human Chronic Gonadotropin HCG Beta • Chromium (blood) • Lead, industrial whole blood • ZPP, Blood photoporphorins, zinc • Paper records printed and shipped (per location) • Make-up physical via local clinic • CD of Digital X-Ray • Manpower Reporting • Lipid Panel The magnitude of this project is between $25,000 and $100,000. All interested concerns will provide this office, in writing, a notice stating their positive intention to submit a proposal as a prime contractor no later than 28 Jan 2019. This notification, which shall not exceed five typewritten pages, must include: (1) A positive statement of intent to bid as a prime contractor (2) A completed and signed Source Sought Information Request Form (3) A listing of projects completed during the past three years, both for government and private industry. The type of project, dollar value, contract number, location, and Point of Contact are to be included. Indicate if you were prime or subcontractor, and if subcontractor, provide the name and point of contact for the prime contractor. (4) A brief outline of resources, subcontractors, and key personnel that would be used to accomplish the contract. In the event adequate Small Business contractors are not available for adequate competition for this project, it may be advertised as unrestricted.
