This notice constitutes Amendment 0002 to the referenced Request for Quotation (RFQ). The purpose of this amendment is to update the submission deadline and to clarify mandatory requirements for all offerors. The solicitation is hereby amended as follows: 1. Revised Submission Deadline The deadline for the receipt of Best and Final Offers (BAFOs) is now 9:00 AM Eastern Standard Time (EST) on 12 January 2026. Quotes received after this time will be considered late and may not be accepted. 2. Mandatory Requirements for Responsible Offeror Determination To be deemed a responsible offeror and for a quote to be considered technically acceptable, the following documentation must be submitted in its entirety with your quotation package. Requirements and Descriptions Insurance & Maintenance Plan - Offerors shall provide proof of liability insurance. Additionally, a comprehensive maintenance and repair plan must be submitted, detailing the scope of included services and guaranteed maximum response times. Provider & Subcontractor Disclosure - Offerors must identify the legal name and physical address of the direct equipment provider. If any services will be subcontracted, a complete list of all subcontractors must be disclosed in the quote. Foreign Vendor Representation - To mitigate risk and ensure timely contract administration, any foreign or overseas-based offeror must provide verifiable contact information for a designated domestic (U.S.-based) representative. This representative must be empowered to act on the offeror's behalf for all matters concerning this solicitation. Signed Amendment - A signed copy acknowledging receipt of this Amendment (Amendment 0002) must be included with the submitted quote. Please be advised: Failure to provide any of the information detailed above will result in a determination that the quote is non-responsive and therefore ineligible for award.
