To provide all labor, materials, and services to perform the requirements as stated in the attached Performance Work Statement. Utilize the incorporated Estimated Test Per Location Breakdown sheet to formulate your quote. Please include per location prices with the estimated tests per location and the cost to complete all the estimated test for the entire base year. Also include a minimum of 1 walk in clinic address for each JOA region. Please complete and submit one sheet for the base year, option year 1, and option year 2. This sheet is incorporated in the RFQ and is included in the SAM. gov posting for convivence. All questions must be submitted in writing via an email to andrew.e.emrich.civ@army.mil not later than 13 April 2024 at 11:00 am EDT. Basis of award will be the lowest price that meets all the requirements of the Solicitation and Performance Work Statement. Wage Determination 2015-4143 Revision 26 dated 26 December 2023 applies and is incorporated. All quotes must be submitted by email to andrew.e.emrich.civ@army.mil.
