Please include per location prices with the estimated tests per location and the cost to complete all the estimated test for the entire base year. Please complete and submit one sheet for the base year, option year 1, and option year 2. This sheet is incorporated in the RFQ and is included in the SAM.gov posting for convivence. Basis of award will be the lowest price that meets all the requirements of the Solicitation and Performance Work Statement.
