AMENDMENT 0001 ***The purpose of this amendment is to extend the due date for offers from 23 February 2024 at 3pm EST to 28 February 2024 at 3pm EST. Please include a signed copy of this amendment with your quote.*** The NYARNG has a requirement for Three (3) Advanced Life Support services (ALS) (Dedicated, On-Site ALS Ambulance). The contractor shall provide three (3) dedicated ALS Ambulances with Crew. The ALS Ambulance Crew must consist of one (1) National Certified Paramedic and one (1) National Certified EMT per ambulance for a total of three (3) Certified Paramedics and three (3) Certified EMTs for this requirement. Two (2) of the Ambulances will serve as primaries and one will be in reserve to Joint Base McGuire–Dix–Lakehurst (JB MDL), NJ 08733 Structural Collapse Venue Range on 16-18 April 2024, from 0700-1900 hrs. In the event either of the primary ALS Ambulances and crew leave for any reason the reserve ALS ambulance and crew must be dispatched prior to the 1st crew leaving. **** See attached RFQ for a more detailed description of this requirement.**** All quotes must be in accordance with the Performance Work Statement (PWS). The PWS is incorporated in the attached RFQ. Basis of award is the lowest price that conforms to the requirement of the PWS. Wage Rate Determination for Ocean County, NJ applies and is incorporated in the attached RFQ. This requirement is a small business set-aside. Offerers must have FAR 52.204-26 completed in SAM Reps & Certs or they must return a completed copy of FAR 52.204-26 with their quote.
