The contract... **********Amendment 001 has updated the PWS and extended the offer due date.*********** The NYARNG has a requirement for Advanced Life Support services (Dedicated, On-Site ALS Ambulance). The contractor shall provide a dedicated ALS Ambulance with Crew (1 certified Paramedic and 1 certified EMT) to Fort Indiantown Gap (FTIG), Annville, PA 17038 Structural Collapse Venue Range on 9-13 SEP 2022, from 0700-1900 hrs. In the event the dedicated ALS Ambulance and crew leave for any reason a new ALS ambulance and crew (1 certified Paramedic and 1 certified EMT) must be dispatched prior to the 1st crew leaving. **** See attached RFQ for a more detailed description of this requirement.**** All quotes must be in accordance with the Performance Work Statement (PWS). The PWS is incorporated in the attached RFQ. Basis of award is the lowest price that conforms to the requirement of the PWS. Wage Rate Determination for Lebanon county, PA applies and is incorporated in the attached RFQ. This requirement is a small business set-aside. Offerers must have FAR 52.204-26 completed in SAM Reps & Certs or they must return a completed copy of FAR 52.204-26 with their quote.
